These injections contain glucagon-like peptide-1 (GLP-1), a naturally occurring hormone in the body which works by:

·         Regulating blood sugar levels

·         Slowing gastric emptying

·         Acting on the brain's appetite centres to reduce appetite

How it works

The success of GLP-1 receptor agonists in weight loss lies in their multifaceted approach to regulating hunger, satiety, and metabolism. The key mechanisms by which GLP-1 agonists promote weight loss are:

Appetite suppression

GLP-1 agonists work directly on the brain's appetite regulation centres, specifically the hypothalamus. By activating GLP-1 receptors in the brain, these drugs signal the body that it is full, reducing the sensation of hunger. As a result, individuals tend to consume fewer calories, which contributes to weight loss.

Delayed gastric emptying

Another significant effect of GLP-1 agonists is their ability to slow down gastric emptying, the process by which food leaves the stomach and enters the small intestine. This delay increases the feeling of fullness after eating, making it easier for individuals to stick to a lower-calorie diet.

Reduction in food cravings

Some studies have shown that GLP-1 agonists can help reduce cravings for high-calorie, high-fat, and sugary foods. By modulating reward pathways in the brain, these medications may make it easier for individuals to avoid overeating or binge eating.

Improved insulin sensitivity

For individuals with insulin resistance or type 2 diabetes, GLP-1 agonists help improve insulin sensitivity, which is critical for regulating blood sugar levels. When blood sugar is well-controlled, the body is less likely to store excess calories as fat.

Enhanced fat burning

Some research suggests that GLP-1 agonists may promote fat loss by increasing the breakdown of stored fat (lipolysis) and reducing the production of new fat cells. This could further contribute to the overall reduction in body fat percentage.

Types of weight loss injections offered

We are offering the two GLP-1 agonists that have proven to be the most effective.

Tirzepatide (Mounjaro)

A newer medication, Tirzepatide, acts on both GLP-1 and GIP (glucose-dependent insulinotropic polypeptide) receptors. This dual action enhances its effects on insulin regulation and appetite suppression, making it highly effective for both diabetes and weight loss.  This is the treatment that we would recommend to anyone new to GLP-1 agonists.

Semaglutide (Wegovy)

Semaglutide is one of the most well-known GLP-1 receptor agonists.  We recognise that there will be many people already using this medication so are offering it to maintain continuity.

Eligibility for weight loss injections

Eligibility for weight loss injections, such as semaglutide (Wegovy, Ozempic) and liraglutide (Saxenda), is based on a combination of Body Mass Index (BMI) and the presence of weight-related medical conditions. These medications are generally prescribed when lifestyle changes (diet and exercise) alone have not been sufficient for weight management. Below are the specific criteria for eligibility:

BMI Criteria
BMI of 30 or Higher (Obesity): Individuals with a BMI of 30 or above qualify for weight loss injections as they meet the criteria for obesity.

BMI of 27 or Higher (Overweight) with Comorbidities: If an individual has a BMI between 27 and 29.9 (considered overweight) and also has weight-related health conditions, they may qualify. Common comorbidities include:

Type 2 diabetes
Hypertension (high blood pressure)
Dyslipidemia (high cholesterol)
Obstructive sleep apnea
Heart disease
Nonalcoholic fatty liver disease (NAFLD)

When not to have weight loss injections?

Weight loss injections are not suitable for everyone. There are specific medical conditions, risk factors, and circumstances where these medications should be avoided. Here are the main contraindications and situations when weight loss injections are not recommended:

1. Personal or Family History of Certain Thyroid Tumors

  • Medullary Thyroid Carcinoma (MTC): Individuals with a personal or family history of MTC should avoid these medications, as GLP-1 receptor agonists may increase the risk of developing thyroid tumors.

  • Multiple Endocrine Neoplasia Syndrome Type 2 (MEN2): If you have MEN2, a genetic condition that predisposes you to tumors, these medications are contraindicated.

2. History of Pancreatitis

  • Acute or Chronic Pancreatitis: People with a history of pancreatitis should not use these injections, as they can potentially increase the risk of inflammation of the pancreas.

3. Severe Gastrointestinal Disorders

  • Gastroparesis or Severe GI Motility Issues: Weight loss injections can slow stomach emptying and may worsen conditions like gastroparesis (delayed gastric emptying).

  • Inflammatory Bowel Disease (IBD): Individuals with IBD (such as Crohn's disease or ulcerative colitis) might experience exacerbation of their symptoms.

4. Allergic Reactions or Hypersensitivity

  • If you have a known allergy to GLP-1 receptor agonists or any ingredients in the injection formulation, you should not use these medications.

5. Severe Kidney Disease or Impaired Kidney Function

  • Chronic Kidney Disease (CKD): In advanced stages, these medications can affect kidney function and may not be suitable.

  • Dehydration Risks: Weight loss injections can cause vomiting or diarrhea, which could lead to dehydration and worsen kidney issues.

6. Liver Disease

  • Although not always an absolute contraindication, people with significant liver impairment should use these medications cautiously and under close medical supervision.

7. Pregnancy and Breastfeeding

  • Pregnancy: Weight loss injections are not recommended during pregnancy as they may harm fetal development. They are generally classified as Category X or Category C, indicating risks to the fetus.

  • Breastfeeding: There is insufficient data on the safety of these medications during breastfeeding, so they are typically not recommended.

8. History of Severe Psychological Disorders

  • History of Suicidal Ideation or Severe Depression: Some people experience mood changes on these medications, so those with severe psychological conditions may need to avoid them.

  • Eating Disorders: Individuals with active or a history of eating disorders (e.g., anorexia nervosa or bulimia) should be cautious, as these medications can significantly alter appetite and weight.

9. Concurrent Use of Other Medications

  • Insulin or Sulfonylureas: Using these together with GLP-1 receptor agonists can increase the risk of hypoglycemia, so caution and close monitoring are necessary.

  • Other GLP-1 Receptor Agonists: Combining different GLP-1 medications is not recommended due to the risk of overdose and increased side effects.

10. Children and Adolescents

  • Weight loss injections are not generally approved for individuals under 18 unless specified by guidelines or in cases of severe obesity under specialist care.  We would not offer this service to anyone under the age of 18 at Oakridge Aesthetics, however it may still be available via your GP.

  •

11. Uncontrolled Hypertension or Cardiovascular Disease

  • If you have uncontrolled high blood pressure or severe heart conditions, these medications may not be suitable due to their effects on heart rate and blood pressure.

Possible side effects of weight loss injections?

Weight loss injections have become popular for their effectiveness. However, they come with potential side effects, which can range from mild to severe. Here's a comprehensive list of possible side effects:

Gastrointestinal Issues

- Nausea: The most common side effect, particularly when first starting the medication.

- Vomiting: Often associated with nausea, especially with higher doses.

- Diarrhoea: Can occur as the body adjusts to the medication.

- Constipation: Another common digestive complaint.

- Abdominal pain: Some may experience cramping or discomfort.

Appetite and Taste Changes

- Decreased appetite: An intended effect but can sometimes be extreme, leading to insufficient nutrient intake.

- Altered taste: Some people report a metallic taste or changes in how foods taste.

Hypoglycemia (Low Blood Sugar)

- This is likely if you have diabetes and are already taking medication for it.  If you are already on diabetic medication then it would be more appropriate for you to access this medication from your GP so that closer monitoring can occur.

Cardiovascular Effects

- Increased heart rate: Noted in some patients and could be concerning if it becomes persistent.

- Hypertension: Elevated blood pressure may occur.

Injection Site Reactions

- Redness, pain, or swelling: Localized reactions at the injection site are common.

- Bruising: Sometimes bruising occurs where the injection was administered.

Neurological Symptoms

- Dizziness: Some individuals may feel lightheaded, particularly when their body adjusts to the medication.

- Fatigue: General tiredness is reported, especially at the start of treatment.

Gallbladder Issues

- Cholelithiasis (Gallstones): There is an increased risk of developing gallstones.

- Cholecystitis (Inflammation of the Gallbladder): This could result from gallstone formation.

Pancreatic Concerns

- Pancreatitis: A rare but serious risk. Symptoms include severe abdominal pain, nausea, and vomiting.

Kidney Issues

- Acute kidney injury: In rare cases, dehydration from vomiting or diarrhea can lead to kidney problems.

Thyroid Effects

- Thyroid tumors: There’s a potential risk of medullary thyroid carcinoma.  This was highlighted in animal studies during clinical trials, but has not yet been confirmed in humans.

- Thyroid nodules: Monitoring is often recommended for those with thyroid disease.

Allergic Reactions

- Rash, swelling, or difficulty breathing: Any severe allergic reaction requires immediate medical attention.

Psychological Effects

- Mood changes: Some people may experience changes in mood or symptoms of depression.
